3.5mm jacks on computer sound cards

i am wanting to hook my pc to my stereo and it has a 5.1 sound card. One 3.5 output is for center/sub i was wondering how to wire the 3.5 plug to a set of rca plugs

I have a 7.1 card for my Laptop (Creative Audigy ZS) it has 3 3.5 jacks for all 8 channels (there are two with three channels on one jack). Is there an adaptor to get those to 3 RCAs? I've looked for an adaptor but haven't come across any.
